Individual Supports

These instructional and training supports are provided to adults living independently or with a family. These services are responsive to the individual’s needs and wishes including assistance with shopping, household chores, medical needs, dietary needs, banking and recreational needs.

Adult Respite Services in the home of a Care Provider: Provides short term services where there is an absence or need for relief of those persons who normally provide care for the participant.

Adult Respite Services in the family home or the home of an individual: Provides short term services where there is an absence or need for relief of those persons who normally provide care for the participant.

Adult Companion Services: Supports the individual to successfully reside in his/her home or in the family home with supervision, socialization and non-medical care.

Individualized Home Supports: Provides support for individual to establish, live in and maintain on an on-going basis a household of their choosing that meets their habilitation needs.

Homemaker: Provide assistance with general household tasks where caregiver is absent or unable to manage home and care.

Chore Services: Provides services needed to maintain the home in a clean, sanitary and safe environment when neither the individual, relative, nor other third party is capable or financially responsible for their provision.

Behavioral Supports and Consultation: Improves the individual’s independence and integration in their community by remediating challenging behaviors or acquiring socially appropriate behaviors.

Community/Residential Peer Support Services: Provides support to enhance the skills of the family to assist the waiver participant to function in the community and at home.

Community/Residential Family Training: Provides training to enhance the skills of the individual to function in the community or family home.

Children’s Respite Services in the family home: Provides short term services where there is an absence or need for relief of those persons who normally provide care for the participant.

Family Navigation: Provision of unbiased expert information and referral supports that are designed to assist families to identify needs and to facilitate and gain access to local generic supports through coordination between family and other service providers.
